What do we offer?

We focus on encouraging independence, socialising and building friendship networks, improving health through exercise and nutrition, and support through advocacy and peer connections.

• exercise and leisure activities
• transport planning and training
• information and referral
• linking to training and employment services
• housing information and tenancy information
• domestic assistance
• linking to parenting and family education

Who is eligible for this service?

People with a mental illness, living independently or transitioning into the community, who have a commitment to actively participate with a recovery-focused service.

We work with clients to develop a recovery plan with goals based on cultivating hope, personal responsibility, and an active sense of self determination; identifying areas of their lives that they wish to change or take more control over.

How much will it cost?

This program is funded by Queensland Health through their Home and Community Care Services.
Contributions are greatly appreciated and donations to Communify Qld are tax deductable.

Your application will be considered against current vacancies and if a position is unavailable you will be placed on a waiting list.
