We are working to help you get housed and stay housed.

We believe that housing is a basic human right and we help people to find the home that is right for them.

Am I eligible?

We work with adults and families who are homeless or at risk of homelessness.

We can help if you are:

  • sleeping rough
  • staying with friends, family or “couch surfing”
  • sleeping in a car, caravan park or low-cost hotel
  • staying in other temporary accommodation
  • staying in emergency accommodation or a refuge
  • at risk of being evicted due to rent arrears, mortgage stress or other factors

How do I access HART4000?

The first step is having an appointment with one of our Homelessness Intervention Workers. Email us at info@hart4000.org.au or attend our drop in service from 1pm (Monday to Friday) at Maida Lilley Community Centre: Level 1, 5 Green Square Close (corner Alfred & Constance Streets). You can also visit us at other neighbourhood centres at other times.

If you are currently in crisis, please come to our drop in service or call Homeless Hotline on 1800 474 753.

What will happen in the appointment?

Making an appointment will allows us to understand more of what is happening for you and the type of support we can offer. In this appointment, we will complete some paperwork and have you sign some consents. We will then be able to link you in with the most appropriate team at HART4000 to commence support.

We offer a personalised response to your housing needs:

  • Finding sustainable housing that is suitable for you
  • Housing advocacy and support
  • Case coordination and planned support to help achieve your housing goals
  • Direct referrals to emergency accommodation providers
  • Referrals and assistance with community and social housing providers including applications, support letters, or bond loan

We can also provide information and referrals to other services:

  • Tenant advice and advocacy services
  • Culturally specific services
  • Youth services
  • Drug and alcohol services (including rehab and detox)
  • Legal advice
  • Immigration advice
  • Counselling
  • Financial counselling
  • Emergency relief agencies
  • Health services
  • Disability services
  • Centrelink
  • Family support services

Opening hours

Monday to Friday, 9am – 5pm (closed public holidays)

HART 4000 has a drop in service that runs from 1pm. You must arrive at 1pm to be seen on that day.
